The right candidate will have high integrity, ambition, and is passionate about providing quality care with a patient-centric team.THIS IS A REMOTE POSITION IN ORLANDO, FL. Job Responsibilities: The LPN will function as a critical member of the Clinical Team and will report directly to the Heart Failure Provider and will work closely with the interdisciplinary team caring for a large cohort of heart failure patients. This LPN will be responsible for providing nursing care for patients enrolled in the Heart Failure clinic. This includes, but may not be limited to:
  • Oversight of the remote patient monitoring program. This includes monitoring vital readings in the dashboard, responding to alerts and messages in a timely manner, with accurate documentation.
  • Maintaining a patient roster in the remote patient monitoring dashboard, including enrollment, tracking device delivery, patient onboarding and setup, and providing guidance to patients on proper usage of equipment. Collaborating with the technical support team for device issues and patient concerns.
  • Recognition of critical alerts with an understanding of the escalation protocol and notifying the provider in a timely manner.
  • Works alongside the care team with patient care coordination as needed, such as following up on patient messages, provide patient wellness calls, and providing patient education materials.
  • Working in close alignment with the clinical team to ensure a seamless patient experience throughout the program.
  • Guiding patients on signs and symptoms of exacerbation, identifying assistance with medication management (escalate to provider) and advising patients how to follow their heart failure action plan.
  • Aim to deliver an optimal patient experience through patient follow-up, education, and service.
Qualifications:
  • Current, Florida LPN license
  • Graduate from an accredited program
  • Minimum 1-2 years experience as a LPN, preferably in cardiology
  • Strong interpersonal skills to communicate effectively and pleasantly with patients and other care team providers.
  • Ability to interpret remote patient monitoring data to ensure appropriate follow ups in a timely manner; ability to enforce accurate and timely documented interventions and referrals.
  • Excellent at taking initiative, multi-tasking, organization, problem-solving, and decision-making skills is a must to be successful in this role.
  • Proficiency in working with electronic health records, such as Athena, as well as tools including Microsoft Word, Outlook, and Excel applications is preferred.
