Humana Inc. (NYSE: HUM) and Cardiovascular Associates of America (CVAUSA), have entered into a groundbreaking multi-state value-based care agreement. This partnership between Humana and CVAUSA’s Novocardia Care Solutions division aims to enhance care for eligible Humana Medicare Advantage Plan members through CVAUSA’s extensive network of cardiovascular practices.
The partnership focuses on providing timely cardiovascular care to improve patient outcomes. By proactively managing chronic conditions like heart failure, the collaboration aims to reduce the total cost of care. Novocardia Care Solutions’ model enhances care coordination, patient experience, and clinical services. The partnership addresses the need for better cardiovascular care by reducing avoidable hospitalizations and emergency room visits, ensuring care is delivered in the most appropriate settings. Both organizations will share performance data to drive ongoing improvements in care quality.
