Nurse Practitioner or Physician Assistant – Cardiovascular Care (North Chicago Nursing Home Focus) - New Grads Welcome
We are looking for a dedicated Advanced Practice Provider (NP or PA) to join our growing team, delivering compassionate cardiovascular care to patients in nursing homes and post-acute care settings across the North Chicago area.

What You'll Do:
Provide direct cardiovascular care in nursing home and extended care environments
Work collaboratively with cardiologists, nursing staff, and facility care teams
Conduct assessments, develop treatment plans, monitor progress, and coordinate follow-up care
Educate patients and families on condition management and preventive strategies
Position Details:
Location: Nursing homes and rehab centers in North Chicagoland and surrounding areas
Schedule: Monday through Friday (day shifts only)
On-Call: None required
Employment Type: Full-time, W2
Compensation: Highly competitive salary + full benefits (health, retirement, PTO, CME, etc.)
Preferred Background:
Experience in cardiology, geriatrics, or internal medicine is ideal
Certified and licensed as a Nurse Practitioner or Physician Assistant in Illinois
A passion for providing high-quality, patient-first care
Able to work independently and within a multidisciplinary team
Comfortable with travel to multiple local facilities
